Reading to Kids Donor Sheet: Click here for a paper sheet you can print and use to track donations from family and friends.  It also includes a poster with information about Reading to Kids (click here for a poster with color photos).  Once you have collected your donations, please mail your donor form and checks to Reading to Kids, 1600 Sawtelle Blvd., Suite 210, Los Angeles, CA  90025.
